It is engineered with amino-acid substitutions and a fatty-diacid lipidation that enables reversible albumin binding — the same albumin-tethering approach used in GLP-3 and GLP-2 — which extends its circulating half-life relative to native amylin or pramlintide. In the developers' rat pharmacokinetic work the half-life is on the order of a day (roughly 20 to 27 hours); the once-weekly profile (a half-life of 159–195 hours) is a finding from studies in people (Enebo et al., Lancet 2021) and does not describe research models.

Cagrilintide selectively engages the amylin receptor family — AMY1, AMY2, and AMY3 — which are heterodimers of the calcitonin receptor (CTR) paired with receptor activity-modifying proteins (RAMP1, RAMP2, and RAMP3 respectively).
